No family should be
separated from a loved one
during a medical crisis
because of the cost or
complexity of travel.

Some things should simply never be a barrier to care. Financial hardship should not determine who gets to sit beside a loved one in their final days. Distance should not decide who receives the best treatment available. And the complexity of travel should not fall on a family that is already carrying everything.

The Hoʻi Mai Foundation will exist to remove those barriers for families that need it most.

And that work has already begun.

THE ENTERPRISE FUNDS THE MISSION

A model where

Hoʻi Mai Travel — our boutique medical travel concierge — is the commercial engine that makes the Foundation possible. Beyond the Foundation's philanthropic funding, the LLC has committed to directly sponsoring five qualifying families per year through fully coordinated medical travel at no cost.

This is not charity in isolation — itʻs a model designed to sustain itself. Every membership directly expands access for a family who would otherwise go without.

Companion travel is often out-of-pocket — adding $500–$1,500 per person.

Lodging — almost never covered

Hospital-adjacent stays can range from
$150–$400 per night — often for days or weeks at a time.

Ground transport — rarely coordinated

Rides between airport, hotel, and hospital can add hundreds per trip — especially in unfamiliar cities.

The full picture — almost never

No single resource coordinates everything. Families piece it together alone — during the hardest moments of their lives. A single trip can exceed $5,000+ in non-medical costs alone. We aim to change that.

THE GAP WE EXIST TO CLOSE

The system helps.
But it doesnʻt carry the whole family.

Some resources exist for families navigating medical travel. Insurance may cover a flight. A nonprofit may cover airfare for children. But the full picture — lodging, ground transport, and caregiver travel often fall entirely on families — adding thousands in unexpected costs.

Lodging, ground transportation, and caregiver travel can easily add up to $3,000–$8,000+ per trip.

A family managing a terminal diagnosis or repeated treatment cycles needs more than a plane ticket. They need one call that takes care of everything — for every treatment, every visit. That is what the Hoʻi Mai Foundation is being built to provide.

Funding

Covering flight, lodging, and ground transportation costs for patients and families traveling for medical care who cannot afford the full cost of service. Inter-island and US Mainland. No family should have to choose between being there and paying rent.

Reunification

Funding rapid-response travel for families facing end-of-life situations or acute medical emergencies — ensuring loved ones can be present when it matters most, regardless of financial circumstance. Expedited review. Immediate response.

Caregiver
Resources

Wraparound support for family caregivers managing long-distance medical care — coordination assistance, local resource connections, and support for the people who show up every time, often at great personal cost.

We are not waiting for perfect conditions to begin.

The Mālama Series is Hoʻi Mai's subsidized program for qualifying families — providing the same thoughtful, full-service medical travel coordination to those who cannot afford the cost of service. We do not wait for the Foundation to be formally launched to begin this work.

The pilot program is active now. If you are a case manager, social worker, or healthcare professional working with families who face barriers to medical travel, we want to hear from you.

The Mālama Series provides full travel concierge coordination at no cost. Travel expenses are covered through sponsorship and partner support whenever possible.

We are actively seeking airline, hotel, and ground transport partners — and individual sponsors — to help fund the travel costs for our first five pilot families. Your support means a family gets there.

ELIGIBILITY GUIDELINES — PILOT PROGRAM
Medical necessity

Documented need requiring travel outside the home community for care not available locally. Confirmed appointment or treatment plan with a licensed provider.

Financial need

Assistance is available to families experiencing financial hardship who are unable to cover the cost of medical travel. Specific eligibility criteria are being developed as the Foundation establishes its formal grant policy. If cost is a barrier, we encourage you to reach out — we will work with you directly to assess fit.

Geographic scope

Priority to Hawaiʻi-based families traveling inter-island or to the US Mainland. All ages. All diagnoses. Not disease-specific.

Medically cleared to travel

Patient must be cleared for commercial travel. Not limited to pediatric patients — families of all ages considered.

Not covered

Elective procedures, medical equipment or supplies, clinical services of any kind, international travel.
